Recent CBSM updates
In November 2021, DHS made updates to the following pages in the CBSM:

Family training and counseling | We added information about the life-sharing matching process to CBSM – Family training and counseling. |
Family residential services | We added information about life-sharing ongoing support to CBSM – Family residential services. |
Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older | We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of Nov. 1, 2021. |
Waiver, AC and ECS general processes and procedures | We updated the 65 years of age section of CBSM – Waiver, AC and ECS general processes and procedures to add a link to the new exception request form, Reopening someone who is 65 years or older to a disability waiver, DHS-7759D. This form applies to scenario 2 for returning to a disability waiver after age 65. |
Certified RSC-TCM private providers | We updated CBSM – Certified relocation service coordination – targeted case management (RSC-TCM) private providers to make the following changes: |
ICS | We updated the definition of integrated community supports (ICS) setting on CBSM – ICS to align with the waiver plan. This update did not change existing policy. |
How to exchange private and protected information via email with DHS | We corrected the email address listed on CBSM – Resource: How to exchange private and protected information via email with DHS. |
Self-advocacy scenarios | We created a new resource, CBSM – Self-advocacy scenarios, for case managers and people seeking self-advocacy skills by using the family training and counseling service. Case managers can use this page as a support tool when developing robust support plans, while considering all additional services available to a person. We also updated the definition of self-advocacy in the glossary. |
Crisis respite | We comprehensively reviewed CBSM – Crisis respite and made the following changes: These updates did not change existing policy. |
Paying relatives and legally responsible individuals | We updated the unallowable section of CBSM – Paying relatives and legally responsible individuals to clarify which services relatives, legally responsible individuals and non-professional guardians and conservators can provide. This update did not change existing policy. |
Individualized home supports | We made a technical correction to the provider standards and qualifications section of CBSM – Individualized home supports (Waiver Reimagine streamlined service). This update did not change existing policy. |
Exclusions from Chapter 245D licensure | We made technical corrections to CBSM – Exclusions from Chapter 245D licensure to add policy about individualized home supports. This update did not change existing policy. |
Training
AASD
AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

Lead agency staff can take the on-demand HCBS disability waiver foundations course to learn the basics about the four disability waivers, including eligibility and covered services.
