In May 2022, DHS made updates to the following pages in the CBSM:

Information about update

ICS settings

We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of May 1, 2022.

Self-advocacy training and support

We updated CBSM – Self-advocacy training and support to explain:

  • · What self-advocacy training and support is
  • · How to authorize self-advocacy training and support through a variety of methods
  • · How to find a self-advocacy training and support provider
  • · When self-advocacy training and support is an option.

  • SILS

    We updated CBSM – Semi-independent living services (SILS) and made the following changes:

  • · Clarified that self-advocacy training for people currently receiving SILS can be covered under certain circumstances
  • · Clarified that SILS may not be used to pay for training, services or items that are available for free in the community
  • · Clarified that SILS may not duplicate resources available from lead agencies or at no cost from non-profit organizations.
  • EIDBI benefit

    We comprehensively reviewed CBSM – Early Intensive Developmental and Behavioral Intervention (EIDBI) benefit and made the following changes:

  • · Clarified information in the eligibility and referral section about the comprehensive multi-disciplinary evaluation (CMDE)
  • · Added a link to MN Autism Resource Portal – Pathway to services and supports
  • · Clarified information in the covered services section about location of services, telehealth and connection between other services
  • · Added instructions to find EIDBI providers.
  • Corporate foster care moratorium liaison contact list

    We updated information for Freeborn County on CBSM – Corporate foster care moratorium liaison contact list.

    Temporary waiver exits: MMIS actions

    Lead agency employment liaison contact list

    We posted a new page, CBSM – Lead agency employment liaison contact list, to provide contact information for lead agencies that have established employment liaisons. To establish or change a lead agency’s liaison, use the Add or update E1MN employment liaison form.

    We updated CBSM – Lead agency employment liaisons to add a link to the new page, CBSM – Lead agency employment liaison contact list.

    FSG

    We comprehensively reviewed CBSM – Family Support Grant (FSG) and clarified content throughout the page.

    Customized living (including 24-hour customized living)

    We updated the authorization, rates and billing section of CBSM – Customized living (including 24-hour customized living) to add information about the Elderly Waiver (EW) 24-hour customized living rate floor.

    Training

    AASD

    AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, go to TrainLink and search for course code “Housing_CM”.
