Overview

The Disability Services Division (DSD) Response Center provides support to lead agency staff who administer DSD home and community-based services, programs and waivers.

How we can help

We provide:

  • · Case consultation
  • · Policy clarification and interpretation
  • · Technical assistance.
  • We also can connect the lead agency to its Regional Resource Specialist (RRS).

    How to contact us

    Submit inquires using the DSD Contact Form and include:

  • · Name and phone number (required)
  • · Explanation of the issue or question (required)
  • · Identifying information for case consultations (e.g., Person Master Index [PMI] number, person’s date of birth, service agreements, waiver type, provider name, etc.)
  • · Explanation of how you have tried to resolve the issue or answer the question already (including other resources you have used, information you have found and tentative answers)
  • · Other agencies involved (e.g., an ombudsman’s office).
  • If you cannot use the DSD Contact Form, call 651-431-4300 or 866-267-7655 (toll free). We are available 9 a.m. to 4 p.m. Monday through Friday.

    Private data

    RRSs

    The RRSs are part of the Response Center. For more information, refer to CBSM – DSD RRSs.
