Updates to MnCHOICES Help Desk Contact Form

On Thursday, May 11, 2023, DHS updated the MnCHOICES Help Desk Contact Form. DHS made changes to coincide with the launch of the MnCHOICES revision. Users will continue to use the form to submit issues related to MnCHOICES 1.0, and they also will begin using the form to submit issues related to the MnCHOICES revision.

When using the updated form, users must enter their MnCHOICES user ID to submit questions.

Other form changes allow users to:

  • · Choose the application (legacy or revision).
  • · Choose the environment (training or production).
  • · Ask about reports and data.
