In April 2023, DHS made updates to the following pages in the CBSM.

Information about update

Corporate foster care moratorium liaison contact list

We updated information on CBSM – Corporate foster care moratorium liaison contact list for Carver County.

Lead agency employment liaison contact list

We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Isanti
  • · Otter Tail
  • · Yellow Medicine.
  • CAC Waiver

    We updated CBSM – Community Alternative Care (CAC) Waiver to add integrated community supports (ICS) to the list of covered services. For more information, refer to the Dec. 30, 2022, eList announcement.

    DD Waiver

    We updated CBSM – Developmental Disabilities (DD) Waiver to make the following changes:

  • · Added an eligibility requirement. For more information, refer to the Jan. 26, 2023, eList announcement.
  • · Added ICS to the list of covered services. For more information, refer to the Dec. 30, 2022, eList announcement.
  • Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

    We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of April 1, 2023.

    ICS settings

    We updated the list of settings on CBSM – Resource: ICS settings.

    Certified RSC-TCM private providers

    We updated CBSM – Certified relocation coordination – targeted case management (RSC-TCM) private providers to add the following new providers:

  • · Bright Horizon Therapy Center
  • · Community Outreach Services.
  • Temporary waiver exits and restarts: MMIS actions

    AC and EW budgets and case mix caps

    We updated CBSM – Alternative Care (AC) and Elderly Waiver (EW) budgets and case mix caps to make the following changes:

  • · Added more detailed information about personal care assistance (PCA) enhanced rates/budgets.
  • · Added a link to CBSM – Assessment applicability and timelines for information about functional needs updates.
  • Remote support

    We updated CBSM – Remote support to add information about consumer directed community supports (CDCS). This policy page replaces the temporary COVID-19 policy for CDCS.

    Training

    AASD

    AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
