Brochures and videos about how to get help with MnCHOICES assessments

DHS collaborated with Queen Health Services to create new and updated brochures and videos. We created these materials to help people:

· Access MnCHOICES assessments.

· Understand MnCHOICES assessments.

· Know what to expect during an assessment.

Brochures

Community members reviewed MnCHOICES assessment: How to get help, DHS-7283-ENG (PDF), and DHS revised it based on their feedback. The brochure is available in the following languages in DHS – Searchable document library (eDocs):

DHS recommends lead agencies share the brochures and videos with people:

· During the intake process.

· When they schedule a MnCHOICES assessment.

· On their social media pages. To request key messages to post on social media with the videos, refer to the MnCHOICES Help Desk Contact Form, DHS-6979 and select other under question type.
