Purpose: To announce the federal Centers for Medicare & Medicaid Services (CMS) approved remote annual assessments for Community First Services and Supports (CFSS)
Effective: July 1, 2025

Remote CFSS annual assessments approved by CMS

CMS approved remote CFSS annual assessments conducted by lead agencies.

Effective July 1, 2025, lead agencies may substitute remote annual assessments conducted by interactive video or telephone for two consecutive in-person CFSS assessments followed by an in-person assessment in the third year.

DHS recommends lead agencies use interactive video to allow for observation when available.

Applicability

The policy change only applies to CFSS annual assessments. There is no change to the remote reassessment policy for:

  • · Alternative Care (AC).
  • · Brain Injury (BI) Waiver.
  • · Community Alternative Care (CAC) Waiver.
  • · Community Access for Disability Inclusion (CADI) Waiver.
  • · Developmental Disabilities (DD) Waiver.
  • · Elderly Waiver (EW).
  • · Essential Community Supports (ECS).
  • Note: Counties that contract CFSS assessments to an external provider cannot complete remote annual assessments.
