We updated information on CBSM – Transition services provider contact list for the following providers:

    We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Stearns.
  • Customized living (including 24-hour customized living)

    We made the following changes to CBSM – Customized living (including 24-hour customized living):

  • · Clarified information about case mix for 24-hour customized living eligibility.
  • · Updated information about wipes for continence care.
  • · Clarified information about collocated settings for BI and CADI.
  • · Clarified information about heightened scrutiny requirements for AC, ECS and EW.
  • · Added a new section about assisted living license portability.
  • · Clarified information about authorization, rates and billing.
  • Housing resources

    We comprehensively reviewed CBSM – Housing resources and made the following changes:

  • · Updated information about Housing Benefits 101 (HB101), HB101 Vault and HB101 Places.
  • · Added information about Section 811 Project-Based Rental Assistance Program for Supportive Housing.
  • · Added information about the Minnesota Aging & Disability Resources find housing tool.
  • · Added a section about resources to pay for moving expenses.
  • · Removed all references to Housing Stabilization Services.
  • Temporary waiver exits and restarts: MMIS actions

    We comprehensively reviewed CBSM – Temporary waiver exits and restarts: MMIS actions and made the following changes:

  • · Removed outdated information about COVID-19.
  • · Updated the links to the DD Waiver scenarios.
  • Day services needs determination

    We updated CBSM – Day services needs determination and made the following changes:

  • · Removed references to adult day services because the needs determination process no longer applies to development of adult day centers, per the 2025 directive from the state legislature.
  • · Removed references to day training and habilitation (DT&H) because the service changed to day support services in 2021.
  • · Clarified the definition of community-based day services.
  • · Updated information about lead agency responsibilities to reflect the temporary licensing moratorium effective Feb. 1, 2026.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, go to TrainLink and search for course code “Housing_CM”.
