Purpose: To share new steps to determine age-related eating dependencies for children who receive CADI, CAC, BI waiver services and use CDCS as a service option

New process to determine eating-related ADL dependencies for children who use CDCS

After a comprehensive review of the determination process, DHS is updating how children are assessed for eating-related needs. Until DHS makes the required technical changes, lead agencies will need to contact DHS when a child experiences a case-mix change as result of needing constant supervision and/or assistance in setting up meals, (i.e., cutting meat, pouring fluids, etc.) AND all of the following:

  • · Uses the BI, CAC or CADI waiver
  • · Is between 61 months and 18 years old
  • · Is assessed using either the MnCHOICES Assessment tool or the LTCC legacy assessment forms.
  • Lead agency action

    Lead agencies should contact DHS via the DSD Response Center to report the change in case mix when the child meets the above criteria.

    DHS action

    DHS will manually adjust the LTCC screening document in MMIS file to reflect the case-mix change.

    This process will be in place until DHS completes updates to the following:

  • · The MnCHOICES Assessment tool
  • · Minnesota Long Term Care Consultation Services Form: Supplemental Form for Assessment of Children Under 18, DHS-3428C (PDF)
  • · MMIS.
  • When the system updates are complete, DHS will notify lead agencies via another eList message.
