The DHS Licensing Division launched new online information for home and community-based services to be licensed under Minnesota Statutes, chapter 245D effective Jan. 1, 2014. The information along with licensing requirements is available on the DHS Licensing Home and Community-Based Services page.

HCBS providers need to review this information, learn about the license application process and apply for a 245D license if they intend to provide 245D licensed services on or after Jan. 1, 2014.

License application process

July 1, 2013 - Process begins for providers who do not hold a 245B license.
Aug. 19, 2013 - Process begins for providers who hold a 245B license.

Failure to submit a timely application will delay licensure and completion of the Minnesota Health Care Programs record review.
