Subject: DHS seeks qualified 245D providers with immediate capacity to serve people with complex needs
Effective date: Immediately
Purpose: Announce DSD asks certain types of 245D-licensed providers to identify themselves if they have immediate capacity to serve people with highly complex behavioral and/or medical support needs

DHS asks 245D providers to let DHS know if they have immediate capacity* to serve people with highly complex behavioral and/or medical support needs. We want to hear from these provider types:

  • · Residential services providers
  • · Home-based habilitation services providers
  • · Supported living services providers.
  • Lead agency role

    DHS will create a list to share with lead agencies. Lead agencies should use this list to identify providers who can serve people with highly complex behavioral and/or medical support needs.

    Qualifications

    * DHS defines immediate capacity as providers who have a sufficient number of current employees with the training and experience to use person-centered thinking and positive support practices. Providers also must meet one or more of the following conditions:

  • · Current or anticipated vacancy in a 245D-licensed foster care home. The vacancy cannot be due to service suspension or termination.
  • · Willingness to request a temporary fifth bed variance for a 245D-licensed foster care home from the DHS Licensing Division.
  • · Ability to provide habilitation and supported living services to people living in their own homes.

    If you have immediate capacity to serve someone with highly complex needs, please complete DHS 245D Licensed Provider Identification and follow instructions to submit to DHS.
