Subject: New requirement for 245D licensed residential providers

Purpose of email: Notify 245D licensed residential providers that as of Aug. 1, they are required to notify the DHS commissioner in writing when issuing a temporary service suspension or service termination to a person receiving residential services.

Effective Aug, 1, 2015, 245D licensed providers must notify the DHS commissioner in writing when they issue a notice of temporary service suspension or service termination from residential services. Providers who issue temporary service suspension or service termination notices dated Aug, 1, 2015, or later must fax the notices to 651-431-7406. (This is a secure fax number.)

DHS Licensing will update the sample policies and forms on the DHS 245D HCBS licensing Web page to reflect the change. The sample forms will include the fax number to send a copy of the notice to the DHS commissioner.
