BI, CAC and CADI Conversion/Diversion

BI- Neurobehavioral (NB) Conversion

A person is eligible for BI-NB Waiver conversion when the person:

  • · Enrolls in the BI-NB Waiver immediately upon discharge from a long term care hospital having received neurobehavioral services
  • · Was a resident of the long term care hospital receiving neurobehavioral services
  • · Received Medical Assistance for the long term care hospital neurobehavioral services for at least 30 days prior to accessing the BI-NB Waiver
  • Bethesda Rehabilitation Hospital in St. Paul is the only hospital licensed in Minnesota to provide in-patient neurobehavioral services for people with severe behaviors.

    BI- Nursing Facility (NF) Conversion

    A person is eligible for a BI-NF Waiver conversion when the person:

  • · Enrolls in the BI-NF Waiver immediately upon discharge from a nursing facility
  • · Was a resident of the nursing facility and
  • · Received Medical Assistance for the nursing facility services for at least 30 days prior to accessing the BI-NF Waiver
  • CAC Conversion

    A person is eligible for a CAC Waiver conversion when the person:

  • · Enrolls in the CAC Waiver immediately upon discharge from a hospital
  • · Was a resident of the hospital before discharge and
  • · Received Medical Assistance for the hospital services for at least 30 days prior to accessing the CAC Waiver
  • CADI Conversion

    A person is eligible for a CADI Waiver conversion when the person:

  • · Enrolls in the CADI Waiver immediately upon discharge from a nursing facility
  • · Was a resident of the nursing facility before discharge and
  • · Received Medical Assistance for the nursing facility services for at least 30 days prior to accessing the CADI Waiver
  • Diversions

    A person is eligible for a disability waiver diversion when the person does not meet the criteria for a disability waiver conversion. This includes people who meet waiver eligibility criteria and who are discharged from all other types of institutions or treatment facilities, such as Regional Treatment Centers (RTCs), Rule 36 Facilities, Rule 5 Facilities.
