New and updated MnCHOICES communication forms/documents

To prepare for the forthcoming MnCHOICES revision in April, DHS has updated or posted the following forms/documents:

  • · MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF): Updates include a new document title to align with the summary option soon-to-be available in the revised MnCHOICES application. (Note: Managed care organizations do not use this form).
  • · MnCHOICES Communication Form, DHS 6791E and its instructions, DHS-6791F (PDF): Lead agencies use the 6791E form to coordinate communication surrounding assessment. Updates include adding care coordinator language. Care coordinators can use the new version to contribute information to an assessment conducted by another lead agency.
  • · MnCHOICES Cover Sheet, DHS-6791K (PDF): Lead agencies may use this new document as a cover letter when MnCHOICES materials require hand corrections. It explains what the person may see and the rationale for the changes to the document(s). It only should be sent when documents include manual edits.
  • New assessment content

    DHS also has developed/revised public versions of the following and made them available in eDocs:

  • · MnCHOICES Assessment Content, DHS-6791J (PDF).
  • · Health Risk Assessment Content, DHS-6791H (PDF).
  • These two documents are intended to be informational only. Do not use them to conduct an HRA or MnCHOICES assessment. Also, these are not intended to be sent to the person before the assessment interview.

    Downloading eDocs forms

    When using PDF eDocs forms, make sure to download the new versions directly from the provided links or from the DHS eDocs library. It’s always best to first save the forms to your desktop and use your Adobe Acrobat or Reader client to open/edit/modify. Do not complete or fill-out forms inside of your internet browser window. That can corrupt the forms.
