Recent CBSM updates

In June 2023, DHS made updates to the following pages in the CBSM.

Information about update

Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

We updated CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to make the following changes:

  • · Updated the list of providers to reflect provider enrollment status as of June 1, 2023.
  • · Added information about the customized living moratorium.
  • ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    Corporate foster care moratorium liaison contact list

    We updated information on CBSM – Corporate foster care moratorium liaison contact list for Becker County.

    Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for Meeker County.

    Collocated setting language (various pages)

    We updated collocated setting information on the following pages:

  • · CBSM – Adult day services
  • · CBSM – Adult day services bath
  • · CBSM – Family adult day services
  • · CBSM – Community residential services
  • · CBSM – Customized living (including 24-hour customized living)
  • · CBSM – Day support services
  • · CBSM – Family residential services
  • · CBSM – Prevocational services.

    ICS

    We updated CBSM – ICS to make the following changes:

  • · Clarified that ICS is a habilitation service.
  • · Added new policy related to minimum capacity of ICS settings (i.e., three or more living units).
  • · Updated collocated setting information.
  • This update includes changes made to the federal Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC) and Developmental Disabilities (DD) waiver programs.     Moratorium on corporate foster care and community residential setting development

    We updated information about exceptions on CBSM – Moratorium on corporate foster care and community residential setting development to reflect legislative changes.

    Disability waiver programs (various pages)

    We comprehensively reviewed and clarified information on the following pages:

    Family caregiver services – AC, ECS and EW

    We comprehensively reviewed and updated the following pages:

  • · CBSM – Family caregiver services – Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW)
  • · CBSM – Caregiver counseling – AC, ECS and EW
  • · CBSM – Caregiver training – AC, ECS and EW.

    Certified RSC-TCM private providers

    We updated CBSM – Certified relocation coordination – targeted case management (RSC-TCM) private providers to make the following changes:

  • · Updated the contact, phone, email, recertification date and locations for ARC Northland.
  • · Updated the contact, address and email for Global Health Link.
  • · Updated the address and recertification date for Oak Tree Support Services.
  • · Updated the contact, address, phone, fax, email and recertification date for Phoenix Service Corporation.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
