Purpose: To clarify what assisted-living facilities qualify as “community residences” for people who use MHM

How to identify assisted-living facilities that are MHM-qualified community residences
DHS updated MHM Policy Manual – Eligibility and enrollment to clarify the definition of an MHM-qualified community residence. If an assisted-living facility is licensed by the Minnesota Department of Health (MDH), that facility qualifies as an MHM-qualified community residence. To confirm that a facility is licensed by MDH, the transition coordinator or lead agency staff member should use the MDH Health Care Provider Directory.
Before moving to the new residence, the transition coordinator must submit Moving Home Minnesota Communication Form, DHS-6759H (select “Assurance of Qualified Community Residence” as the reason for communication) to tell DHS which community the person chooses to transition to.
Background
People enrolled in MHM must transition to a qualified community residence to maintain eligibility to receive 365 days of MHM services in the community. A qualified community residence should honor personal choice and control of the person’s home and give them opportunities for independence and community integration.
