Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

This page is a point-in-time resource for lead agencies and customized living providers who serve people on the Brain Injury (BI) and Community Access for Disability Inclusion (CADI) waivers. DHS updates this page monthly.
The page includes a list of current, DHS-enrolled customized living provider settings that are only allowed to serve people on the BI and CADI waivers who are age 55 and older. Customized living settings currently in the DHS enrollment process are not included in the list, including settings moving to new locations or experiencing a change in ownership.
To verify if a customized living setting not on this page is enrolled with DHS, review MinnesotaHelp.info or the provider’s record in Medicaid Management Information System (MMIS). Settings not enrolled with DHS are ineligible to receive customized living services authorizations.
Policy
The BI and CADI waiver customized living setting location and age limitations, as explained on CBSM – Customized living, apply to customized living and 24-hour customized living service settings that were operational on or after Jan. 11, 2021.
The age limitation is programmed in MMIS to prohibit customized living service authorizations for people younger than age 55 if the provider’s DHS HCBS provider enrollment record indicates their setting was operational on or after Jan. 11, 2021.
