This page is a point-in-time resource for lead agencies and providers that lists all collocated HCBS provider settings with a submitted HCBS settings rule provider attestation from 2017 through 2018 or an approved site-specific review. DHS updates this page monthly. To confirm a setting is enrolled, search MinnesotaHelp.info.

Collocated setting policy
Any service setting developed after Jan. 1, 2023, must comply with collocated setting policies for services through:
When a single provider leases or owns more than one service setting located on the same or adjoining property, the lead agency can only authorize services in one of the settings.
A service setting includes a setting used to deliver any of the following services:
HCBS settings with provider attestations
Settings DHS determined compliant with the HCBS settings rule regulations during the provider attestation process may continue to provide services in settings that are collocated and operationally related. This page contains the list of all collocated HCBS provider settings with submitted HCBS settings rule provider attestations for the BI, CAC, CADI and DD waivers.

Providers must complete a site-specific review for collocated settings that meet any of the following criteria:
Providers must complete a site-specific review to ensure the people living in these settings are not isolated from the community. To begin the site-specific review process, providers must use the DSD Contact Form.

The following settings submitted an HCBS settings rule provider attestation from 2017 through 2018 or have an approved site-specific review. These settings are allowed to have BI, CAC, CADI or DD waiver services authorized with the collocated setting.
The “Provider setting name” is the name the organization submitted in the provider attestation from 2017 through 2018 or an approved site-specific review. DHS did not make changes to the setting name, unless it only included numbers.
If a provider believes their service setting is listed in error or is incorrectly named, they can contact DHS using the DSD Contact Form.
