New MHM supplemental services available July 1, 2025

Effective July 1, 2025, new MHM supplemental services are available to support a person’s transition to the community. The new supplemental services are one-time services not covered by Medicaid.

DHS posted new pages in the MHM Program Manual for all new supplemental services. For information specific to each service, refer to:

  • · Community education and integration costs.
  • · Environmental modifications deposit.
  • · Pantry stocking.
  • · Pre-transition clean-up services.
  • · Pre-transition non-medical transportation.
  • · Records and fees.
  • · Transition integration fund.
  • Note: Effective July 1, 2025, the MHM Demonstration and Supplemental Services Table (PDF) is no longer available. For policy information about each MHM service, refer to the MHM Program Manual service pages.

    Service availability

    Effective July 1, 2025, the new MHM supplemental services are available for people who are enrolled in MHM and:

  • · Have fee-for-service coverage.
  • · Are enrolled in Special Needs BasicCare (SNBC).
  • · Are enrolled in Families and Children (i.e., Prepaid Medical Assistance Program [PMAP]).
  • Note: Effective Aug. 1, 2025, the supplemental services will be available to people enrolled in Minnesota Senior Health Options (MSHO) and Minnesota Senior Care Plus (MSC+). This timeline will give managed care organizations adequate time to prepare for the launch, as required in their seniors managed care contract.

    Provider enrollment

    Before providers deliver the new MHM supplemental services, they must enroll with Minnesota Health Care Programs (MHCP). To enroll, providers must either:

  • · Register for the online Minnesota Provider Screening and Enrollment (MPSE) portal.
  • · Complete MHCP Home and Community-Based Services (HCBS) Programs Service Request, DHS-6638 (PDF).
