Waiver Reimagine survey: DHS seeks input on disability waiver reform

DHS seeks input on disability waiver reform through a voluntary survey. We value feedback from the community as we align the goals of disability programs and services with the goals of the community.

The survey includes questions about:

  • · How people get information about disability waiver services.
  • · Future changes to disability waivers.
  • · Individualized budgets.
  • · Choice and control over disability waiver services.

    Use the following information to complete the survey:

  • · Survey available: April 30, 2024
  • · Completion deadline: May 30, 2024
  • · Anonymity: Responses will remain anonymous. DHS will not collect personal information.

  • Background information

    Waiver Reimagine is DHS’ ongoing work to make system-level improvements to Minnesota’s four disability waiver programs. This work will reform home and community-based service authorization and delivery.

    During Phase 1 of Waiver Reimagine, we streamlined and simplified the waiver service menu across the four current disability waivers:

  • · Brain Injury (BI) Waiver.
  • · Community Access for Disability Inclusion (CADI) Waiver.
  • · Community Alternative Care (CAC) Waiver.
  • · Developmental Disabilities (DD) Waiver.
  • During the 2021 first special session, the Minnesota Legislature authorized DHS to implement Phase 2 of Waiver Reimagine to support and expand informed choices and informed decision-making through the following tasks:

  • · Reshape the four current disability waiver programs into a two-waiver structure.
  • · Move people who use disability waivers to individualized budgets.
  • · Increase self-directed services access and options.
