Long-term services and supports (LTSS) encompasses a range of health and social services that enable people to choose when and how they receive support. Access to most services and supports are determined through an assessment process. The assessment helps people determine public programs that might pay for services and learn about other options if they do not qualify for publicly funded programs. When a person receives publicly funded services, the lead agency works with the person to develop a person-centered support plan. The plan reflects the person’s needs, preferences and goals. It supports informed decision-making about the services and supports that help the person live and participate in their community.
