Purpose: To notify providers that DHS updated the Positive Support Transition Plan (PSTP) Instructions

Positive Support Transition Plan (PSTP) Instructions updated

DHS recently updated the PSTP Instructions, DHS-6810B, (PDF). The changes made the instructions easier to follow and understand. There have not been any policy changes.

Providers use the PSTP Instructions to complete the:

  • · Positive Support Transition Plan, DHS-6810 (PDF)
  • · Positive Support Transition Plan Review, DHS-6810A (PDF)
  • · Behavior Intervention Reporting Form (BIRF), DHS-5148.
  • These transition plan processes are in place to guide providers as they:

  • · Prevent people who receive services from physically harming themselves or others
  • · Eliminate the use of prohibited procedures
  • · Avoid the emergency use of manual restraint
  • · Increase people’s quality of life.
