In September 2019, DHS made updates to the following pages in the Community-Based Services Manual (CBSM):

Information about update

Specialized equipment and supplies

We updated CBSM – Specialized equipment and supplies and made the following changes:

  • · Added policy for used equipment
  • · Added “evaluation of the person’s need for the equipment and/or device” and “equipment rental in a trial period” to covered services for the Elderly Waiver (EW)
  • · Explained criteria through which the service can cover oral nutritional products for EW and Alternative Care (AC).
  • Support planning

    We updated CBSM – Support planning for long-term services and supports to clarify lead agency responsibilities about record keeping.

    RSC-TCM private providers

    We updated information for the following providers on CBSM – Relocation service coordination – targeted case management (RSC-TCM) private providers:

  • · Capital Health Services
  • · Care Community Services
  • · Independent Living Placement.
  • PCA and CSG enhanced rate/budget process

    We updated CBSM – Personal care assistance (PCA) and Consumer Support Grant (CSG) enhanced rate/budget process to add additional instructions for counties/tribal nations to request an enhanced budget for eligible CSG participants at reassessment or initial assessment.

    Corporate foster care moratorium liaison contact list

    We updated contact information for Aitkin County on CBSM – Corporate foster care moratorium liaison contact list.

    Family training and counseling

    We comprehensively reviewed CBSM – Family training and counseling – BI, CAC, CADI and DD and made the following changes:

  • · Clarified the service definition
  • · Grouped covered services for training into the following categories: training for the person, training for the person and/or family and person-centered planning facilitation
  • · Clarified that services cannot replace or duplicate services available through other funding sources
  • · Grouped provider standards and qualifications for training into the following categories: qualified professionals, classes, courses or conferences and person-centered planning facilitation
  • · Added information about background studies
  • · Added information about service amount to match existing waiver plan language.
  • EW conversion rates

    We created CBSM – Elderly Waiver (EW) conversion rates to explain the process lead agencies should follow for people on EW leaving nursing facilities if the monthly EW budget caps are insufficient to support the services they need to transition to community living.

    Forms for LTSS assessment, eligibility and support planning

    We comprehensively reviewed CBSM – Forms for long-term services and supports (LTSS) assessment, eligibility and support planning and made the following changes:

  • · Combined information from a disability-specific waiver forms page and a MnCHOICES CountyLink resource
  • · Added information for long-term care consultation (LTCC), AC, EW and PCA
  • · Added a column to indicate which forms are used with MnCHOICES.
  • Public guardianship

    We updated CBSM – Public guardianship with information about the updated form, Annual Review of Ward under Public Guardianship, DHS-5836. The lead-agency-delegated public guardian can now save DHS-5836 to a computer and come back to it, as needed.
