Waiver Reimagine survey: DHS seeks your input on disability waiver consolidation

DHS seeks your input on the plan to consolidate the current four disability waivers into two waivers. Go to Waiver Reimagine survey: Your feedback on the transition to a two-waiver system to tell us your thoughts about the changes. The survey will be open from July 13 through July 27, 2022.

DHS will use information gathered in this survey to develop policies to reshape the four waivers into two waivers during Phase 2 of Waiver Reimagine. If you need support or have questions related to the survey, contact the Waiver Reimagine via the DSD Contact Form.

Background and resources

Waiver Reimagine is the name of ongoing work DHS is doing to make system-level improvements to Minnesota’s four disability waiver programs. Phase 1 of Waiver Reimagine involved streamlining and simplifying the waiver service menu across the four current disability waivers:

  • · Brain Injury (BI) Waiver
  • · Community Access for Disability Inclusion (CADI) Waiver
  • · Community Alternative Care (CAC) Waiver
  • · Developmental Disabilities (DD) Waiver.
  • During the 2021 first special session, the Minnesota Legislature authorized DHS to implement the next phase of the Waiver Reimagine project to support and expand informed choices and informed decision-making. Phase 2 includes reshaping the four current disability waivers into a two-waiver structure:

  • · Individual Supports Waiver to serve people living independently or at home with family.
  • · Residential Supports Waiver to serve people living in paid residential settings, such as community residential settings or customized living settings.
