To: People using consumer directed community supports (CDCS) and the Consumer Support Grant (CSG), people interested in information about Community First Services and Supports (CFSS) and other interested parties

Purpose: To announce contracted financial management services (FMS) providers

DHS signs contracts with agencies to provide FMS

DHS contracted with these 16 organizations to provide FMS in Minnesota:

· Accord

· Accra Consumer Choice

· Acumen Fiscal Agent LLC

· Alliance Health Services FMS

· Best Care FMS

· Consumer Directions Inc.

· GT Independence

· Heritage Home Health Care Inc.

· Life Fountain Home Health Care Inc.

· Lifeworks Services Inc.

· Mains’l Financial Management Services

· MRCI (Mankato Rehabilitation Center) Client Directed Services

· Minnesota Quality Care Inc. (Aspen FMS)

· Orion Intermediary Services Organization (ISO)

· Partners in Community Supports Inc. (PICS)

· RedOne Fiscal Agent Inc.

Information about CDCS and CSG

As before, people who use CDCS and CSG:

· Must use an FMS provider.

· May select any FMS provider contracted with the state.

· May change FMS providers if they want.

People who use CDCS and CSG do not need to take any action right now unless their current FMS provider will no longer be contracted with the state.

Information about CFSS

When CFSS begins, a person in these groups must select an FMS provider from the list of contracted organizations:

· Person who uses the CFSS budget model.

· Person who uses the CFSS agency model and wants to buy goods and services.

A person who selects the CFSS agency model and does not want to buy goods and services will not need to select an FMS provider.
