Location: Room 2370 of the Elmer L. Andersen Human Services Building, 540 Cedar St., St. Paul, MN 55101.

Technology: The first few minutes of every meeting will be dedicated to addressing any connectivity issues to ensure everyone can hear other meeting participants.

Public comments: The committee encourages public participants to share their thoughts and ask questions about committee activities at the end of each meeting. Questions and comments can be emailed to TransitionsAdvisoryCouncil.DHS@state.mn.us.

Meeting purpose: The advisory council will work together to recommend the supports needed for people who are experiencing barriers transitioning from hospitals and other settings into their home and community.
