Recent CBSM updates

In January 2024, DHS made updates to the following pages in the CBSM.

Page

Information about update

Adult foster care

We updated CBSM – Adult foster care and made the following changes:

  • · Clarified information in the non-covered services section about what is not covered within the scope of the service and what is not covered when the service is being used.
  • · Added environmental accessibility adaptations (EAA) to the list of services that cannot be authorized for people receiving adult foster care.
  • · Updated the secondary information section to include HCBS Final Rule requirements in 42 CFR 441/301(c).
  • · Updated statute references in the setting license requirements for community residential setting (CRS) requirements.
  • This update includes changes made to the federal Elderly Waiver (EW) program. For more information, refer to June 27, 2023, eList announcement: EW renewal approved by CMS.

    Chore services

    We updated the secondary information section on CBSM – Chore services to add that if the person also receives individual community living supports (ICLS), their support plan must be specific enough to ensure there is no service duplication.

    This update includes changes made to the federal EW program. For more information, refer to June 27, 2023, eList announcement: EW renewal approved by CMS.

    ICLS

    We updated CBSM – ICLS and made the following changes:

  • · Added Minn. Stat. §256S.13 to the legal authority.
  • · Updated the covered services section to add that people receiving ICLS services must receive assistance and/or support in a minimum of two of the six ICLS service components.
  • · Updated the non-covered services section to add that ICLS does not cover ICLS services that do not include a minimum of two ICS components
  • · Updated the non-covered services section to add that ICLS does not cover services that duplicate authorized state plan or other waiver services the person already receives.
  • · Updated the process section to add information about case manager/care coordinator responsibilities for people currently receiving ICLS services to ensure they meet the requirement of at least two ICLS service components.
  • · Updated ICLS Planning Form, DHS-3751 to reflect that people receiving ICLS services must receive assistance and/or support in a minimum of two of the six ICLS service components.
  • This update includes changes made to the federal EW program. For more information, refer to June 27, 2023, eList announcement: EW renewal approved by CMS.

    Transitional services (EW)

    We updated CBSM – Transitional services (EW) and made the following changes:

  • · Updated the eligibility section to change citations for licensed settings.
  • · Updated the non-covered services section to add that transitional services do not cover supports or items that are covered under other state plan or waiver services the person already receives.
  • This update includes changes made to the federal EW program. For more information, refer to June 27, 2023, eList announcement: EW renewal approved by CMS.

    EW conversion rates

    We updated CBSM – EW conversion rates and made the following changes:

  • · Updated the determination section because traditional and consumer directed community supports (CDCS) services now follow the same process and use the same request form.
  • · Updated information in the initial approval section.
  • · Added links to the additional resources section.
  • Home-delivered meals

    We updated CBSM – Home-delivered meals and made the following changes:

  • · Updated the covered services section to add required criteria that must be met when authorizing home-delivered meals in customized living settings.
  • · Updated a citation for state and local health regulations and ordinances related to food preparation, handling and serving.
  • This update applies to BI, CAC, CADI, DD and EW programs. It also reflects changes from the EW renewal. For more information, refer to June 27, 2023, eList announcement: EW renewal approved by CMS.

    Respite

    We updated CBSM – Respite and made the following changes:

  • · Updated the citation for the adult family foster care setting license.
  • · Updated the unlicensed setting for adults section to add that the case manager must determine the setting will meet the assessed needs of the person and document the assessment in the person’s support plan.
  • · Updated the authorization, rates and billing section for BI, CAC, CADI and DD because 15-minute respite is now a market rate service.
  • This update applies to BI, CAC, CADI, DD and EW programs. It also reflects changes from the EW renewal. For more information, refer to June 27, 2023, eList announcement: EW renewal approved by CMS.

    Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Clay.
  • · Ramsey.
  • · Winona.
  • ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

    We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of Jan. 1, 2024.

    DWRS

    We comprehensively reviewed CBSM – Disability Waiver Rate System (DWRS) and made the following changes:

  • · Moved respite (15-minute) to the market rate services list (previously in the framework services list).
  • · Moved homemaker cleaning to the pre-determined rate services list (previously in the market rate services list).
  • Market rate services (BI, CAC, CADI and DD)

    We comprehensively reviewed CBSM – Market rate services (BI, CAC, CADI and DD) and made the following changes:

  • · Removed homemaker cleaning from the list because homemaker cleaning is no longer a market rate service.
  • · Removed the “daily” reference from respite because all types of respite are now market rate services.
  • Homemaker

    We updated the authorization, rates and billing section on CBSM – Homemaker to reflect:

  • · Homemaker cleaning is no longer a market rate service.
  • · Lead agencies can authorize homemaker cleaning at the pre-determined, 15-minute rate.
  • Customized living

    We updated CBSM – Customized living (including 24-hour customized living) and made the following changes:

  • · Added EAA to the list of services that cannot be authorized with customized living (EW only).
  • · Added information to the secondary information section about home-delivered meals in customized living settings.
  • · Added information to the secondary information section about the Assisted Living Report Card.
  • · Added information about additional provider requirements for customized living providers who deliver transportation.
  • This update applies to BI, CADI and EW programs. It also reflects changes from the EW renewal. For more information, refer to June 27, 2023, eList announcement: EW renewal approved by CMS.

    Specialized equipment and supplies

    We updated CBSM – Specialized equipment and supplies and made the following changes:

  • · Updated the amount of specialized equipment and supplies a person may receive per year under BI, CAC and CADI in the service amount section.
  • · Added information to the secondary information section about the item description to include in a claim.
  • Guide to support a person with a residential service termination notice from a 245D-licensed provider

    We updated and expanded CBSM – Guide to support a person with a residential service termination notice from a 245D-licensed provider to include specific pages about:

  • · Overview of the process for residential service termination.
  • · Case manager responsibilities and legal rights of the person or their legal representative.
  • · Waiver service options to consider as additional supports.
  • · Non-waiver service options to consider as additional supports.
  • · Mental health service options to consider as additional supports.

    Training

    AASD

    AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS).
