MnCHOICES revision application user survey
DHS asks all MnCHOICES revision application users to complete a survey to share feedback about their experience with the application, communication and engagement with DHS.
To complete the survey, refer to PCG – MnCHOICES revision survey. The deadline to complete the survey is Tuesday, March 5, 2024.
DHS will use the survey results to evaluate:
MnCHOICES revision launch schedule
DHS is currently in Phase 2 of the partner-advised rolling schedule of the MnCHOICES revision. For more information, refer to the Nov. 9, 2023, eList announcement.
