Phase 4 of the MnCHOICES revision launch will begin on July 1, 2024
On Monday, July 1, 2024, DHS will start Phase 4 (the final transition period) of the extended partner-advised rolling launch of the revised MnCHOICES application, which DHS first announced in the Nov. 9, 2023, eList.
The rolling launch allows lead agencies to gradually:
DHS will deactivate the legacy systems at the end of the rolling launch. Effective Oct. 1, 2024, users will no longer be able to access the legacy systems.
Phase 4 (final transition period)
This phase is scheduled to run from July 1 to Sept. 30, 2024, during which lead agencies should:
Effective Oct. 1, 2024, legacy systems, including MnCHOICES 1.0, will no longer be available.
Exception: Six counties currently contract with public health nurse agencies that use the legacy personal care assistance (PCA)/Community First Services and Supports (CFSS) document to conduct PCA/CFSS reassessments. DHS will work with these counties to move their work to the MnCHOICES revision after Sept. 30, 2024.
Terminology
Legacy systems: MnCHOICES 1.0 assessment and support plan applications, legacy assessment and support planning tools and other assessment/support planning tools used by managed care organizations (MCOs).
MnCHOICES revision: The vendor-hosted version of MnCHOICES that will replace all legacy systems.
New assessments: Assessments completed for people who have not had an assessment using a legacy system.
