Olmstead disability inclusion and choice survey

The Olmstead Implementation Office seeks feedback about disability inclusion and choice through a survey. The survey asks questions about community integration, choice and inclusion. Anyone can take this survey, but the main audience is people with disabilities in Minnesota.

The Olmstead Implementation Office will use the information collected through the survey to inform focus areas for Minnesota’s next Olmstead Plan.

Use the following information to complete the survey:

· Survey: To take the survey, refer to Olmsted survey on disability inclusion and choice.

· Deadline: June 30, 2024.

· Available languages: English, Spanish, Hmong and Somali.

More information

Over the coming year, the Olmstead Implementation Office will provide other opportunities for people to share input that will affect the next Olmstead Plan.
