Purpose: To announce a change to allow transitional services as a covered service under Alternative Care (AC)

Transitional services available under AC

Effective March 1, 2025, DHS will allow lead agencies to add transitional services to AC support plans and service agreements.

DHS made this change because:

  • · The 2024 Minnesota Legislature added transitional services as a covered service under AC. For more information, refer to Laws 2024, chapter 127, article 47, section 12, subdivision 5.
  • · The Centers for Medicare & Medicaid Services (CMS) approved the change on Jan. 2, 2025. For more information, refer to the Feb. 4, 2025, eList announcement.
  • Background information

    Transitional services provide items and supports necessary for a person to move from a licensed setting to independent or semi-independent community-based housing. Previously, transitional services were not a covered service under AC, and lead agencies used AC discretionary services to meet people’s needs.

    DHS actions

    DHS must update our policy manuals, forms and affected systems as applicable.

    We plan to implement changes to:

  • · Medicaid Management Information System (MMIS) on Feb. 6, 2025.
  • · MnCHOICES on Feb. 7, 2025.
