Grant opportunity to reduce hospital discharge delays for medically complex pediatric patients

DHS is accepting applications from licensed home care nursing agencies to develop a pilot program that addresses hospitalization and discharge delays for medically complex pediatric patients.

Through the implementation of the Pediatric Hospital to Home pilot program, DHS will collect data to better understand the challenges associated with delayed discharge for children who are medically complex.
