CFSS implementation updates

DHS began the transition from PCA and CSG to CFSS on Oct. 1, 2024. We will provide regular updates throughout the transition.

Policy manual

DHS updated CFSS Manual – Assessment for PCA/CFSS services and CBSM – CSG transition to CFSS with instructions to authorize additional PCA/CSG for people who have not completed the transition to CFSS by their next assessment.

Forms and documents

DHS updated:

  • · Information for People Eligible for CFSS Services, DHS-6893U (PDF) with more details for people eligible for CFSS. We are working to update the translated versions of this form. Lead agencies must discard previous versions of this form and use the 7-25 version.
  • · Lead Agency Addendum to CFSS Individual Service Delivery Plan, DHS-6893W with additional fields for people who receive CFSS services on a waiver/Alternative Care (AC) with consumer directed community supports (CDCS) or home care nursing and resolved an issue with electronic signatures.
  • DHS updated CFSS Individual Service Delivery Plan, DHS-6893P to:

  • · Add the ability to select the starting month in the month-to-month general plan for the agency model.
  • · Add a field that calculates the total amount available for budget model workers.
  • · Correct the payroll tax amount for budget model workers.
  • · Add a drop-down menu to enter instrumental activities of daily living (IADLs).
  • · Add the ability to indicate informal caregivers will perform IADLs.
  • · Clarify instructions for people who use the agency model to leave the classes and individual abuse prevention plan (IAPP) sections blank.
  • · Update the budget model summary table to ensure the lead agency allocates the person’s entire budget.
  • · Remove the lead agency signature field. Lead agencies must sign Lead Agency Addendum to CFSS Individual Service Delivery Plan, DHS-6893W to document their approval of the plan and the final authorization amounts.
  • MMIS instructions

    DHS updated DSD MMIS Reference Guide – Type B service agreement (SA) for PCA/CFSS to add instructions on how to use the COPY FROM feature to save time when they enter a type B PCA/CFSS SA.

    MMIS system updates

    DHS continues to test and fix bugs in MMIS. Counties/tribal nations can use Service Agreement and Screening Document (SASD) Support Team Portal, DHS 3754 to report issues to DHS.

    Resources

    DHS fixed an error in the CFSS budget model calculator that caused it to incorrectly round some large budgets to one digit instead of two digits, and the resulting entry caused MMIS to produce the over cap error in some cases.
