In September 2025, DHS made updates to the following pages in the CBSM.

Information about update

Transition services provider contact list

We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Independent Living Placement.
  • · Love Investments.
  • Crisis respite

    We comprehensively reviewed CBSM – Crisis respite and made the following changes:

  • · Clarified information throughout the page.
  • · Added information about using Extension Request for Out-of-Home Waiver Crisis Respite in MDH-Licensed Hotel, DHS-8633.
  • · Added information about using Transition Summary and Plan After Out-of-Home Crisis Respite, DHS-8633A, which is a template that includes all required components.
  • Corporate foster care moratorium liaison contact list

    We updated information on CBSM – Corporate foster care moratorium liaison contact list for the following counties:

  • · Nicollet.
  • · Olmsted.
  • ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    IAR for HCBS

    We updated CBSM – Initial assessment review (IAR) for home and community-based services (HCBS) to clarify the applicability, purpose and limitations of the IAR.

    Notice of action

    We comprehensively reviewed CBSM – Notice of action and made the following changes:

  • · Added definitions for denial, termination and reduction.
  • · Added information about Assessment Summary: Action Taken by the Lead Agency and Your Right to Appeal, a MnCHOICES form that replaced DHS-2828A.
  • · Added information about Notice of Action: Explanation of Changes to Your Support Plan, a MnCHOICES form that replaced DHS-2828B.
  • · Clarified information about timelines.
  • · Explained that counties/tribal nations can use DHS-2828A and DHS-2828B when the person’s assessment and support plan are not in MnCHOICES.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, go to TrainLink and search for course code “Housing_CM”.
