Assisted Living Report Card updates
On Oct. 28, 2025, DHS published new ratings and corrected 14 previously published ratings for its Assisted Living Report Card.
New ratings
We published new resident health, safety and staffing ratings from surveys conducted by the Minnesota Department of Health (MDH) between March 1 and May 31, 2025.
Corrected ratings
We corrected the health, safety and staffing ratings for the following assisted living settings:
We corrected these ratings after we identified an error in data previously provided by MDH as part of our ongoing quality assurance efforts. The corrections are due to errors in previously published ratings and do not reflect provider performance.
MDH identified the source of the data error. DHS and MDH are working together to make corrections to the Assisted Living Report Card as needed. DHS will send another eList announcement when we resolve the issue.
Next update
DHS plans to publish the next update to the Assisted Living Report Card in January 2026.
