Assisted Living Report Card updates

On Oct. 28, 2025, DHS published new ratings and corrected 14 previously published ratings for its Assisted Living Report Card.

New ratings

We published new resident health, safety and staffing ratings from surveys conducted by the Minnesota Department of Health (MDH) between March 1 and May 31, 2025.

Corrected ratings

We corrected the health, safety and staffing ratings for the following assisted living settings:

· Brooklyn Park Assisted Living, 7711 Humboldt Ave. N., Brooklyn Park, MN 55444

· Caring Home Health Inc., 2529 17th Ave. S., Minneapolis, MN 55404

· Copperfield Hill – The Lodge, 4020 Lakeland Ave. N., Robbinsdale, MN 55422

· Diamond Willow Assisted Living, 913 Old Highway 2, Proctor, MN 55810

· Ely Carefree Living, 140 S. Second Ave. W., Ely, MN 55731

· Golden Bright Homes LLC, 2842 Raleigh Ave., St. Louis Park, MN 55416

· Golden Nest LLC, 1918 19th Ave. NE, Minneapolis, MN 55418

· Midwest Residential Inc, 10449 Johnson Ave. S., Bloomington, MN 55437

· Oak Meadows Senior Living, 8131 Fourth St. N., Oakdale, MN 55128

· St. John Home Care LLC, 17515 30th Ave. N., Plymouth, MN 55447

We corrected these ratings after we identified an error in data previously provided by MDH as part of our ongoing quality assurance efforts. The corrections are due to errors in previously published ratings and do not reflect provider performance.

MDH identified the source of the data error. DHS and MDH are working together to make corrections to the Assisted Living Report Card as needed. DHS will send another eList announcement when we resolve the issue.

Next update

DHS plans to publish the next update to the Assisted Living Report Card in January 2026.
