EIDBI provisional license application checklist now available
DHS posted EIDBI Pre-Licensing Application Checklist, DHS-8818 (PDF), which explains the information required for the EIDBI provisional license application. We expect the application to be available in mid-January 2026.
EIDBI agency actions
EIDBI agencies can use EIDBI Pre-Licensing Application Checklist, DHS-8818 (PDF) to prepare to complete the online provisional license application. All EIDBI agencies must apply for a provisional license by May 31, 2026.
Agencies that have not submitted an application by May 31, 2026, must not operate, and DHS will disenroll them from providing EIDBI services.
Background information
The 2025 Minnesota Legislature created new provisional licensing requirements that affect all enrolled EIDBI agencies.
