Overview

Minnesota Intensive Therapeutic Homes (MITH) is a program under the DHS Direct Care and Treatment (DCT) Division's Community-Based Services (CBS). MITH services are provided in family foster homes and represent an add-on service, over-and-above regular foster care.

MITH, as well as all other CBS programs, cannot provide hospital level of care for a medical condition or psychiatric illness.

Eligibility

A person is eligible for MITH services if all of the following criteria are met:

  • · The person is younger than age 21
  • · The person is medically stable
  • · The person has a pattern of severe impairment in psychosocial functioning due to a psychiatric illness or intellectual disability that requires intensive treatment home placement
  • · The intensive treatment home identified for the person’s placement has sufficient resources and qualified staff to provide necessary services
  • · The person’s presenting signs and symptoms clearly demonstrate a need for 24-hour structure, supervision and active treatment
  • · Acute inpatient treatment is not required, is inappropriate or has been ruled out
  • · The treatment is not available in a less restrictive setting
  • · The intensive treatment home identified for placement meets the person’s needs, including consideration for cultural and gender-specific preferences.
  • Team

    The MITH program hires one foster parent as a full-time employee and then strives to create and coordinate an effective wrap-around team to support the child/youth and the foster parents. At minimum, the team includes:

  • · Child/youth
  • · Parent(s)/legal representative(s)
  • · Case manager
  • · MITH team members, which include the program nurse, a behavior analyst and a mental health professional/supervisor
  • · Other community supports (e.g., schools, community therapists)
  • · Any other significant people invited by the child/youth or their legal representative and case manager.
  • Services

    MITH services are outlined in the person’s Coordinated Services and Support Plan (CSSP), DHS-6791B and MITH’s corresponding Coordinated Services and Support Plan Addendum (CSSPA). The CSSPA typically outlines supervision needs. It includes a behavior support plan, as well as plans to support the child/youth and foster parent on issues related to health, medication management, etc.

    MITH also offers the following supports to the child/youth and foster parent:

  • · 24/7 access to an on-call professional via phone
  • · Training on care plans, mental health diagnoses, etc.
  • · Coordination of services through regular team meetings.
  • Setting

    MITH provides services in family foster care settings that are not secured facilities.

    Referral

    To refer a person for MITH services, contact one of MITH’s supervising mental health professionals at 612-398-1086.

    Funding

    MITH services are funded through home and community based-services (HCBS) waiver funding approved by the lead agency through a service agreement.
