This page is a tool for lead agencies, case managers and people who use services to find information about approved integrated community supports (ICS) settings, including:

  • · Provider.
  • · Location.
  • · Approved capacity (i.e., the total number of people who can reside in living units and receive ICS).

    • To search for a specific provider on this page, use the keys Ctrl + F (Windows) or Command + F (Mac).

    Policy

    ICS settings are provider-controlled home and community-based services (HCBS) settings. Each setting must have an approved setting capacity report.

    ICS may be provided in:

  • · All the living units in a multi-family housing building that has three or four units.
  • · Less than 25% of the living units in a multi-family housing building that has five or more units.
  • · 25% or more of the living units in a multi-family housing building that has five or more units, only after DHS has approved a site-specific review for that setting.

    Note about Housing Support: ICS settings are eligible for Housing Support. However, not all ICS providers have Housing Support agreements. DHS does not maintain a public list of ICS settings with Housing Support agreements. Lead agencies manage Housing Support for ICS settings. For more information, refer to DHS – Housing Support.

    Case managers must contact the ICS provider for the setting-specific NPI/UMPI number to enter a service authorization. The information on this page might not be up to date between publication dates due to the revalidation process and ongoing program integrity compliance. Case managers must verify a provider is enrolled by checking DHS – MHCP provider directory.
