In December 2023, DHS made updates to the following pages in the CBSM.

ICS settings

We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

Corporate foster care moratorium liaison contact list

We updated information on CBSM – Corporate foster care moratorium liaison contact list for the following counties:

  • · Blue Earth.
  • · Fillmore.
  • · Kandiyohi.
  • · Scott.
  • Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

    We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of Dec. 1, 2023.

    Chore services

    We comprehensively reviewed CBSM – Chore services and made the following changes:

  • · Clarified information in the non-covered services section about what is not covered within the scope of the service and what is not covered when the service is being used.
  • · Clarified that services under all waiver and Alternative Care (AC) programs must meet the requirements listed in the services section of CBSM – Waiver and AC programs overview.
  • · Clarified that if the person also receives homemaker services, their support plan must be specific enough to ensure there is no service duplication.
  • AC fees

    We comprehensively reviewed CBSM – Alternative Care (AC) fees and made the following changes:

  • · Added information about the AC Client Disclosure Form, DHS-3548.
  • · Added additional instructions to the process and procedure section.
  • · Added a section about estate claims.
  • Exclusions from Chapter 245D licensure

    We comprehensively reviewed CBSM – Exclusions from Chapter 245D licensure and made the following changes:

  • · Clarified information throughout the page.
  • · Moved information about provider standards and qualifications to one section to make this information easier to find.
  • Transition services provider contact list

    We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Access Health, LLC.
  • · Gateway Unlimited Living, LLC.
  • · Hope Outreach Solutions, LLC.
  • · Kingsland Care Services Inc.
  • · New Infinite Relocation.
  • · Best Life Relocation Services.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
