To: Moving Home Minnesota (MHM) transition coordinators, lead agency staff, case managers, substance use disorder (SUD) treatment programs, care coordinators and other interested parties

Purpose: To announce the Professional Statement of Need (PSN) form now meets the screening document requirement in the MHM transition services eligibility and enrollment guidelines

PSN form now available for MHM eligibility and enrollment process

Qualified professionals and people seeking MHM transition services can now use the PSN form to meet the screening document requirement in MHM’s eligibility and enrollment process.

The PSN form helps verify:

  • · Eligibility for one or more programs.
  • · Funding/services to help people afford, find and keep housing.
  • Qualified professionals can only use the PSN form for people in a qualified institution that meets all of the following requirements:

  • · A licensed SUD treatment program.
  • · Enrolled in the Minnesota SUD System Reform 1115 Demonstration.
  • · Designated as an institute for mental diseases (IMD).
  • Background on the PSN

    Previously, people used the PSN form to access Medical Assistance (MA) Housing Stabilization Services and Housing Support. As of Jan. 9, 2024, people can also use the PSN form to access MHM transition services.

    This change does not affect the current requirements for people seeking:

  • · Housing Stabilization Services. For more information, refer to DHS – Housing Stabilization Services.
  • · Housing Support. For more information, refer to DHS – Housing Support.
  • · MHM transition services in other qualified institutions. For more information, refer to MHM Program Manual.
