Survey about service suspension and termination form development

DHS seeks feedback from 245D-licensed HCBS providers on the forms to report service suspensions and terminations to DHS. Specifically, the survey seeks provider input on:

  • · DHS’ current process to submit a service suspension or termination for people with disabilities.
  • · Development of a new form to report service suspensions and terminations to DHS.
  • Survey details

    Use the following information to complete the survey:

  • · Survey available: May 28, 2024
  • · Completion deadline: June 18, 2024
  • · Anonymity: Responses will remain anonymous. DHS will not collect personal information.
  • · How to participate: To complete the survey, refer to DHS – Provider survey on the service suspension/termination process.
  • Background information

    Proposed legislation would require all HCBS providers (defined under Minn. Stat. §245D.03, subd. 1, para. C, sect. 3) to use DHS forms to report service suspensions and service terminations. If the legislature passes the initial version of the bill, this change would take effect on Aug. 1, 2024.
