Recent CBSM updates

In April 2024, DHS made updates to the following pages in the CBSM.

Page

Information about update

Crisis respite

We updated CBSM – Crisis respite to add specific instructions about submitting documentation of continued need for crisis respite in a Minnesota Department of Health (MDH)-licensed hotel.

ICS settings

We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

Waiver and AC programs overview

We added a link to How to Apply for a Home and Community-Based Services (HCBS) License Under MN Statute 245D (video) in the additional resources section on CBSM – Waiver and Alternative Care (AC) programs overview.

Specialized equipment and supplies

We updated information about the service amount for Brain Injury (BI), Community Alternative Care (CAC) and Community Access for Disability Inclusion (CADI) waivers on CBSM – Specialized equipment and supplies. For more information, refer to the April 9, 2024, eList announcement.

DD Waiver waiting list guide

We comprehensively reviewed CBSM – Developmental Disabilities (DD) Waiver waiting list and made the following changes:

  • · Removed information about funding limits because limits have changed.
  • · Clarified information throughout the page.
  • ICS transition plans

    We updated CBSM – ICS transition plans and made the following changes:

  • · Clarified the transition process and removed information about housing with services establishments from Scenario 2: Customized living settings in which all people are on BI or CADI waivers.
  • · Removed the collocated customized living setting transition options because the time-limited transition period expired.
  • · Marked all scenarios about supported apartment programs as expired because the transition period for a supported apartment program to an ICS setting no longer applies.
  • ECS

    We comprehensively reviewed CBSM – Essential Community Supports (ECS) and made the following changes:

  • · Updated the organization of the page to align with other CBSM pages.
  • · Updated contact information for questions about ECS service authorization for managed care organizations (MCOs).
  • Waiver, AC and ECS case management

    We updated CBSM – Waiver, AC and ECS case management and made the following changes:

  • · Added Minn. Session Laws 2021, 1st Special Session to the legal authority.
  • · Added definitions of contracted case management, courtesy case management and lead agency.
  • · Expanded on information in the covered services section about informed choice, informed decision-making and coordination with tribal nations.
  • · Added information in the monitoring frequency section about supporting the person’s meeting preferences.
  • · Added information about required trainings for BI, CAC, CADI and DD waiver case managers.
  • · Reorganized content about service requirements, choice and provision of case management, grievances and financial interest.
  • Transition services provider contact list

    We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Promise Comprehensive Services.
  • · Thomas Allen, Inc.
  • Guide for EAA home modifications

    We comprehensively reviewed CBSM – Guide to home modifications under the environmental accessibility adaptations (EAA) service and clarified information throughout the page.

    Corporate foster care moratorium liaison contact list

    We updated information on CBSM – Corporate foster care moratorium liaison contact list for Koochiching County.

    Training

    AASD

    AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
