Purpose: To announce a survey requesting information from CL providers that serve people on the Brain Injury (BI) or Community Access for Disability Inclusion (CADI) waivers

CL survey request for BI and CADI waivers
On July 8, 2024, DHS will send a letter and survey to MN–ITS mailboxes of CL providers that serve people on the BI and CADI waivers. Providers should complete this survey to:
Purpose
DHS plans to use this information to publish a list of active and enrolled CL settings in the Community-Based Services Manual (CBSM). The list will help lead agencies and people who receive services:
