Save information from in-progress online forms before July 12

If you use any of the following forms, you must complete your in-progress versions before July 12, 2024, to avoid losing data:

· Consumer Directed Community Supports (CDCS) Budget Exception Request Application, DHS-6633

· Early Intensive Developmental and Behavioral Intervention (EIDBI) Referral Form, DHS-6751S

· Housing Cohort Status Report Form

· Remote Support Exception Form, DHS-7759A

· Managed Care Organization (MCO) Form to Add a Person to the MnCHOICES Database

· Monitoring Technology Approval Request, DHS-6789A

· OBRA Level II Preadmission Screening for People with Developmental Disabilities Form, DHS-4248

· Reopening Someone Who is 65 Years or Older to a Disability Waiver, DHS-7759D.

Background

DHS will move these forms to a new server in early July. You will lose in-progress information when they move to the new location. DHS will not be able to capture your partial input after the forms move.

How to save information

You will not be able to use the save button on the form to save your input. To save your work, either:

· Print screenshots of your work.

· Copy and paste the information you entered into a new document.

New links

The forms will have new links when they move to the new location. The old links will redirect you to the new versions. Make sure to note the new links and update your bookmarks.
