To: Moving Home Minnesota transition coordinators (MHM TC) and lead agency staff who work with Moving Home Minnesota (MHM)
From: DHS Aging and Adult Services and Disability Services divisions
Purpose: To announce a policy change to allow access to MHM TC services with adult mental health targeted case management (AMH-TCM), children’s mental health targeted case management (CMH-TCM), vulnerable adult/developmental disability targeted case management (VA/DD-TCM), behavioral health home (BHH) or assertive community treatment (ACT) services
Effective: Aug. 1, 2024

Policy changes to allow access to MHM TC with AMH-TCM, CMH-TCM, VA/DD-TCM, BHH and ACT

Effective Aug. 1, 2024, a person may receive MHM TC and the following services in the same month:

  • · AMH-TCM.
  • · CMH-TCM.
  • · VA/DD-TCM.
  • · BHH services.
  • · ACT services.
  • Note: This change does not apply to relocation service coordination targeted case management (RSC-TCM).

    The change to allow a person to receive TCM and MHM at the same time will help Minnesota advance toward person-centered practices in all areas of service delivery.

    Provider actions

    Providers must coordinate all transition and TCM efforts to:

  • · Ensure care continuity.
  • · Prevent duplicate efforts.
  • They are responsible to document the roles and responsibilities for the:

  • · Transition coordinator.
  • · Targeted case manager.
  • · Other professionals who provide support to the person.
  • Background information

    Previously, a person had to choose between TCM and MHM services, even though these are separate services. A person could not bill on the same month for MHM TC service and:

  • · AMH-TCM.
  • · CMH-TCM.
  • · VA/DD-TCM.
  • · BHH services.
  • · ACT services.
