Purpose: To announce a survey for people impacted by EVV

EVV survey

DHS seeks feedback from people impacted by EVV, including:

· Providers.

· Caregivers.

· People who receive services.

· Other interested parties.

Use the following information to complete the survey:

· Survey available: July 30, 2024.

· Completion deadline: Aug. 30, 2024.

· How to participate: To complete the survey, refer to DHS – EVV survey.

· Instructions: Complete the survey once. Do not include any identifying information (e.g., Federal Employer Identification Number [FEIN], National Provider Identifier [NPI], Unique Minnesota Provider Identifier [UMPI]).
