Feedback about barriers to transition from emergency rooms and hospitals

DHS and the University of Minnesota Institute on Community Integration (ICI) seek feedback about barriers to transition from emergency rooms and hospitals. We want feedback from people who live in Minnesota to help us understand challenges people experience when they transition home after being admitted into the emergency room or hospital.
