Purpose: To announce that adults using the Developmental Disabilities (DD) Waiver must now receive materials about how to recognize and report maltreatment

Policy change for DD Waiver maltreatment materials
Effective Aug. 26, 2024, all people age 18 and older using the DD Waiver must receive Minnesota Abuse Reporting Center Brochure, DHS-6778E (PDF) at each MnCHOICES assessment or reassessment. This document gives the person information about how to recognize and report maltreatment.
DHS added a check box to the assessment and program acknowledgement screen in the revised MnCHOICES application for the certified assessor to confirm the person received the brochure. DHS is in the process of updating its manuals to reflect this change.
