DHS publishes CFSS individual service delivery plan template

In preparation for the Oct. 1, 2024, transition to CFSS, DHS published CFSS Individual Service Delivery Plan Template, DHS-6893P. People will use this document to describe what CFSS services they will receive and how those services will be delivered, as described on CFSS Manual – PCA/CFSS service delivery plan.

Note: A county partner reported an issue with how the form currently adds goods and services in the agency model. DHS is working to fix the issue.

We will continue to prepare new forms and update existing PCA forms before implementation. As we publish forms, we will add them to CFSS Manual – Forms and documents.
