In August 2024, DHS made updates to the following pages in the CBSM.

Forms (various pages)

On Oct. 1, 2024, DHS will remove legacy assessment and support planning tools used by lead agencies and other assessment/support planning tools used by managed care organizations (MCOs) from DHS – Searchable document library (eDocs). We will discontinue these documents to make sure all agencies use the MnCHOICES revision. For more information, refer to the Sept. 3, 2024, eList announcement.

We will update CBSM pages on a rolling basis to remove references to these tools.

Lead agency employment liaison contact list

We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Anoka.
  • · Olmsted.
  • · Red Lake.
  • Moratorium on corporate foster care and community residential setting development

    We comprehensively reviewed CBSM – Moratorium on corporate foster care and community residential setting development and made the following changes:

  • · Removed an expired moratorium exception for people receiving customized living or 24-hour customized living services.
  • · Added a new moratorium exception for people affected by the closure of homes with a capacity of five or six beds currently licensed as supervised living facilities (SLFs) under Minn. R. 4665 but not designated as intermediate care facilities (exception available until June 30, 2025).
  • ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    Corporate foster care moratorium liaison contact list

    We updated information on CBSM – Corporate foster care moratorium liaison contact list for the following counties:

  • · Dodge.
  • · Isanti.
  • · Kandiyohi.
  • · Renville.
  • · Steele.
  • · Waseca.
  • MnCHOICES certified assessors

    We comprehensively reviewed CBSM – MnCHOICES certified assessors and made the following changes:

  • · Added a responsibility to develop a person-centered assessment summary that reflects identified needs and support options within the context of values, interests and goals important to the person.
  • · Added a responsibility to provide the person with an assessment summary of findings, support options and agreed-upon next steps.
  • · Updated the credentials and experience section to add that assessors must be employed by a lead agency and meet the education and experience requirements in Minn. Stat. §256B.0911, subd. 13.
  • · Updated information about the required training.
  • · Added additional policy resources.
  • Transition services provider contact list

    We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Ace Community Connect.

  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
